Jacob A. "Jake" Crouch, 29, Moline, passed away on Friday, March 10, 2017 in Davenport. Funeral services are 6PM Thursday at Van Hoe Funeral Home Ltd., East Moline. Visitation is from 4PM until service time. Memorials may be made St. Mary's Moline Food Pantry.

Jake was born on June 25, 1987 in Moline, the son of Jack and Patty (Anderson) Crouch. He attended Moline High School and had a passion for cooking. He had a big heart, enjoyed playing video games, roller blading and loved fishing with his dad.

The sharing of this tragic and heartbreaking loss, is to help spread awareness.

Jacob, who struggled with depression and addiction, off and on, throughout his young adult life, lost his battle to addiction. A disease that is real and can happen to anyone. Jake, who was loved by so many, who only wanted to see him get better, will be missed so very much.

Those left to mourn his passing, his father, Jack Crouch, Rock Island, mother, Patty (Rick) Hernandez, Moline, step sisters, Lindsey (Tim) Palos, Moline, Jenny (Buzz) DeSmith, Geneseo, step brother, Tim (Liz) Hernandez, Bettendorf, special aunt, Pam Boyle, and numerous uncles, nieces, nephews and cousins.

He was preceded in death by his grandparents, Dick and Sharon Anderson and Jack and Lilly Mae Crouch.
